Do these documents need witnesses or a notary?

Most do — the specific requirements are printed on each document itself. A Simple Will needs two witnesses (and a notary for the self-proving affidavit); most Powers of Attorney and deeds need notarization; guardian designations and the medical directive accept either two witnesses or a notary.
